To help streamline your intake process, we will send you a link to your email so you can confidentially register on our portal. Please complete the appropriate documents prior to your visit.

Adult Patients
To provide the highest level of care please bring your most recent lab results and a recent progress note to your visit. Or, once you are registered into our portal, you can upload the documents directly into your account.
Veterans & Military
We are honored to be part of the Community Care Network for Veterans. You need to ask your current provider for a Referral to come see us for Psychiatry.
If your VA provider needs help to schedule you directly with us, please ask them to call (847) 487-6869.
They can also fax documents to (847) 316-9797.
Adolescent Patients
We are ready to listen to you! If you are between the ages of 12 - 16 you can independently request and receive up to 8 outpatient visits without parental consent. We cannot notify your parents without your consent during this initial time period unless deemed absolutely necessary.
If you are 17 or older you can receive outpatient psychiatric services completely without parental consent or notification
